REFORMING NURSING EDUCATION AND PRACTICE IN NIGERIA.

Three Bills that should be presented to the National Assembly in efforts to reform nursing education and practice in Nigeria:

1. NATIONAL COMMISSION FOR COLLEGES OF NURSING (ESTABLISHMENT) BILL - A bill for an Act to establish the National Commission for Colleges of Nursing to, amongst other things, advise the Federal Government on all aspects of nursing education falling outside the universities, and polytechnics and other matters ancillary thereto.

When there is a National Commission For Colleges of Nursing (NCCN), there will be no need for going under the National Board for Technical Education to obtain HND.

2. FEDERAL COLLEGES OF NURSING (ESTABLISHMENT) BILL - A bill for an Act to Establish the federal colleges of nursing listed in the bill and to provide for courses leading to the award of degrees, fellowships, diplomas and certificates in nursing science, midwifery and other related courses; and for other matters conncected therewith.

The Colleges of Nursing shall hold examinations and grant professional diplomas, degrees, certificates and other distinctions to persons who have pursued a course of study approved and accredited by the National Commission for Colleges of Nursing.

3. NURSING AND MIDWIFERY (REGISTRATION, ETC.) ACT CAP. N143, LFN, 2014 (REPEAL AND RE-ENACTMENT) BILL - A bill for an Act to repeal the Nursing and Midwifery (Registration, Etc.) Act CAP. N143, Laws of the Federation of Nigeria, 2004 and enact the Nursing and Midwifery Practice Act, to provide for education program standards, standards and scope of nursing and midwifery practice, Types of titles and licenses, Requirements for licensure, and for related matters.

The need for Legal scope of practice was clearly stated in the IAP Award (1981) as reproduced below:

''An act of the National Assembly giving legal definition and coverage to the duties and functions of Nurses according to the scheme and course content of their training should as a matter of urgency be brought into existence if it had not already been in existence''.

"Nurse" should be defined as a person licensed as an advanced practice registered nurse (APRN), registered nurse (RN), or licensed practical nurse (LPN). Scope of practice should be commensurate with the educational preparation at postgraduate level (for APRN), degree level (for RN), and sub-degree level (for LPN) respectively.

An Advanced Practice Registered Nurse (APRN) is a baccalaureate prepared registered nurse (RN), who has also accomplished a graduate degree in nursing in one of the following four roles of APRN:
*. The Certified Registered Nurse Anesthetist (CRNA)
*. The Clinical Nurse Specialist (CNS)
*. The Certified Nurse Midwife (CNM)
*. The Nurse Practitioner (NP)

In addition to choosing a role, an APRN must choose one of the following patient populations as a focus of their practice (called population foci).
a. Family care without regard to the age of the members of the family;
b. Adult gerontology care;
c. Neonatal care;
d. Pediatric care;
e. Gender-related health care; and
f. Psychiatric and mental health care.

Registered Nurse, Certified (RNC) - this level requires registration with the Council as Registered Nurse as an entry requirement. RNC means a registered nurse who has received certification for practice as a Registered Nurse certified in a specialized field of professional nursing e.g. Registered Nurse, Certified in Neonatal Intensive Care (RNC-NIC). The qualification shall yield no professional registration but can be logged as an additional qualification with the Council. A registered nurse, certified, shall not practice advanced practice registered nursing.

"Registered nurse/Registered Midwife" abbreviated RN/RM means an individual holding dual registration as a registered nurse and a registered midwife. A Registered Nurse/Registerd Midwife (RN/RM) is not an APRN and shall not practice advanced nurse-midwifery or advanced midwifery.

Licensed Practical Nurse (LPN) should be a new role within the health care team, designed to bridge the gap between health care assistants and registered nurses.

"Midwife" should be defined as a person licensed as a Certified Nures Midwife (CNM), Registered Advanced Midwife Practitioner (RAMP), or registered midwife (RM). Scope of practice should be is commensurate with the educational preparation at postgraduate level (for CNM) and (RAMP), and first degree level (for RM)

NANNM is especially well situated to promote effective reforms by collecting and disseminating international best practice and incentivising their adoption.

In order to adopt international best practice, the Nursing and Midwifery (Registration, Etc.) Act. Cap. N143, LFN, 2004 has to be repealed and re-enacted to provide for scope of practice for all tiers and types of nurses/midwives and other related matters.
