Lagos University Teaching Hospital School of Post Basic Nursing

Twelve Months Post Basic Nursing Courses

Application are invited for admission into the underlisted one year Post Basic Nursing Programme, which will commence in November 2015

Peri Operative Nursing
Accident & Emergency Nursing
Pediatric Nursing
Ophthalmic Nursing

REQUIREMENT
Applicants should possess the RN qualification with or without RM, WASC/GCE with 5 credits including English Language, Mathematics and Biology from not more than two sitting. Applicants who qualified from 2006 till date should have credit in English Lang, Mathematics Physics, Chemistry and Biology. Two years post qualifcation experience and current practicing licence issued by the Nursing and Midwifery Council of Nigeria are mandatory


CONDITIONS FOR ADMISSION
Written examination to be conducted on the 13th June 2015 by 9.00 am at school of Post Basic Nursing (Human Resources Development Centre) Lagos University Teaching Hospital.

(LUTH Successful candidates are to bring along originals of all relevant certificate for interview at the same venue from 7th - 10th July 2015


METHOD OF APPLICATION

Each applicant should obtain a bank draft for the sum of N5,000.00 (five Thousand Naira Only in favour of Lagos University Teaching Hospital (LUTH). The draft should be taken to the cash Office LUTH from 9.00am - 2.00pm for exchange of a receipt. Photocopy of the receipt MUST be submitted to the office of the Chief COrrdinator, School of Post Basic Nursing before collection of the FORM

CLOSING DATE FOR RETURN OF APPLICATION FORMS
The copies of the completed application forms, 2 recent passport phorographs, photocopies of relevant certificates and emails address should reach the Administrative Office of the School not later than Friday 5th June 2015

Procedures for payment to obtain forms
Pay the sum of ten thousand naira ony (N10,000 ) to:
Account: LUTH TSA A/C Account Number: 0230137361014. After payment obtain the print out which should be submitted to the cash office from 9.00am - 2.00pm for exchange of a receipt and collect your receipt to obtain the form at
the school.. Kindly endevour to make two copies of the receipt obtained from the cash office which must be submitted to the office of the
Chief Co-ordinator, School of Post Basic Nursing before collection of the FORM. Come along with four (4) passport
photographs.

Candidates are to bring along original copies of all relevant certificates for interview at the same venue from 18th - 21st July, 2017.

METHOD OF APPLICATION:
• Each applicant should obtain a bank draft for the sum of N10,000.00 (Ten thousand naira) only, in favour of Lagos University Teaching Hospital (LUTH).

• Before the payment is made, students are to go to any Cyber Cafe to generate a code/pin For payment at any Bank of choice.

• A print-out is given at the Bank after payment. The print-out is then taken to the Finance and Accounts Department, LUTH, where a receipt will be issued in place of the PRINT-OUT. The photocopy of the receipt issued should be presented to the Deputy Director, Nursing Education.
